Build the production UltraQuant D=256 decode kernel.

padded_slot is the per-(token, head) slot stride in bytes (kv_cache.shape[3], >= 272). Cache layout is [num_blocks, block_size, num_kv_heads, padded_slot] uint8 AoS.

QK is always scaled FP4×E4M3 MFMA; V always uses native CVT + HW transpose; tile-groups are always strided. fuse_qrot selects the in-kernel WHT when the query layout is eligible.
